Welcome to MDChronic
The following Terms & Conditions apply to all services and programs provided by MDChronic to its patients and clients, in conjunction with any relevant quotation, unless otherwise agreed in writing.
These Terms & Conditions (the “Terms”) govern your access to and use of our Services. In accordance with these Terms, our “Services” include our digital healthcare programs (the “MDChronic Programs”) and all relevant services and features we offer.
At any time and for any reason, we reserve the right to amend or replace any of the provisions in this agreement. It is your responsibility to frequently review this agreement for modifications. We will give you notice and specify a reasonable time period before the new terms take effect if we alter or replace this agreement.
You consent to be bound by the revised terms if you keep using or accessing the Services after any changes take effect. You should discontinue using the Services if you disagree with any modifications.
1. Our Services
We offer the MDChronic Programs any associated services and features as part of our services. Our online healthcare programs for those who are at risk for, already have, or want to avoid specific illnesses or injuries. Certain people who satisfy any applicable condition-specific clinical requirements that we have set for enrollment or for particular features are eligible to participate in the MDChronic Programs.
Our Services may include, without limitation:
- Access to information, text, images, resources, or other materials that are displayed in the MDChronic Programs, the Apps, the Sites, or any other part of the Services;
- Creating, uploading, transmitting, displaying, and accessing User Submissions;
- Communicate with our support team and healthcare staff, as well as some of the professionals in our care team;
- Ability to engage with other MDChronic Members and participate in one or more online communities in relation to your health goals;
2. Access To Our Services
Only the following people can access the MDChronic Programs:
- Fulfill the minimum age requirements (at least 18 years old for the other MDChronic Programs and MDChronic for Joint & Muscle Health);
- Fulfill any relevant clinical requirements pertaining to a given condition that we have set for enrollment or for particular MDChronic Program components;
We reserve the right to deny the Services to any individual or organization in our sole discretion. Additionally, we reserve the right to alter any components of the MDChronic Programs or any relevant criteria used to assess clinical eligibility for one or more MDChronic Programs for any reason at any time.
The Services or any part of the Services (including, but not limited to, any feature, specification, database, or content) may be modified, replaced, suspended, or discontinued at any time and for any reason in our sole discretion.
If your rights to access or use the Services are changed, replaced, suspended, or discontinued, we won’t be held responsible. Without prior notice or responsibility, we may also restrict your access to all or a portion of the Services.
3. Restrictions Concerning Medical Care
MDChronic members can interact with our care and support staff and access the information they need. Aspects of the MDChronic programs that call for such a license under applicable law may occasionally be provided to you by members of the care team who hold professional licenses and certifications. We do not provide any Medical advice OR Medical treatment.
Before beginning, altering, or discontinuing any course of medical treatment, therapy, or treatment for any injury or for any questions about your medical condition, diet, or the use (or frequency) of any medication or medical device, always get advice from your physician or other qualified health providers. Do not disregard professional medical advice or put off seeking it because of any information you receive in connection with our services. Additionally, do not use the services as a substitute for speaking with your physician or other health care provider. The services are not intended to replace your own physician’s or qualified health provider’s treatment plans; rather, they are meant to supplement them.
Any advice, treatment or therapy course, diagnosis, or other information, services, or products you may receive from third parties in connection with or as a result of using the services are not our responsibility or liability.
4. No Emergency Services
The services are only intended for non-emergency uses. We do not provide any kind of emergency or time-critical service, including emergency or time-critical communications to any kind of hospital or law enforcement agency.
5. Enrollment
In order to participate in an MDChronic Program, you must first register with us and provide us with information on the Site (or, in certain situations, over the phone or via video). You guarantee and represent that all of the information you provide to us is true, accurate, and comprehensive. You represent and declare that you have examined and verified that the information before submitting it to us, including when you authorize a third party to help you register or apply for an MDChronic Program.
You also commit to keep the data you send us (or any third party sends on your behalf) up to date and to swiftly update it in the event that anything changes. Your account may be terminated immediately if you violate this agreement by failing to keep your information up to date.
6. Consent to Receive Electronic Communication
You must give us your phone number and email address in order to access or use the MDChronic Programs, and some other features of the Services. By using the Services, you accept and consent to us contacting you by phone calls, push notifications, SMS/text messages, email, and other electronic communications pertaining to the Services.
Unless you decide to unsubscribe or modify your communication settings, which you can do at any time, you consent to receiving these messages. You consent that we may call the number you give and leave a voicemail about any concerns relating to MDChronic performance of any health care activities through an MDChronic Program, including calls about my care, insurance issues, reminders, and payment obligations.
You consent to receive all agreements, notices, disclosures, and other communications from us in electronic format as part of using the Services, including those that are mandated by law or these Agreements. Any notice or other information we provide in connection with your application, registration, or enrollment in an MDChronic Program, as well as any communications pertaining to your access to and use of the Services, are subject to your consent to receive electronic notices, disclosures, and other communications.
